Herpes zoster sine herpete - Case report

  Summary:
Herpes zoster is localised sporadic disease characterised by the unilateral radicular pain and vesiculose skin eruption in the dermatome belonging to one spinal or one cranial nerve. It is caused by the endogenous reactivation of Varicella zoster virus infection which is present in a latent form in dorsal roots ganglias. AIM: To point out to the importance of clinical diagnosis and very frequent occurrence with malignant disease in older people. METHOD: We used clinical, laboratory and other relevant diagnostic data from the patient health record card to present the case. CASE REPORT: Female patient MG, 51 years old, came to medical check-up complaining about burning pain localised from the right flank region towards the anterior abdominal region up to the middle line, followed by sharply confined reddish skin, started 10 days earlier. During the follow up, there were no other skin changes in the patient except erythema. Laboratory analyses were done and all the results were within normal ranges. Gastroenterologist did the check-up and abdominal echosonography was done. The diagnosis of Herpes zoster sine herpete was made on the basis of the distinctive, localised, segmental neuralgic pain. THERAPY: Causal antiviral, Aciklovir tbl. 5x800 mg, B group vitamins, analgetics and local zinc lotion. All changes improved on therapy. Soon after therapy, due to breast injury, control mammography was done and there were signs of mild fibrocystic dysplasia. Biopsy was done. Patohistological diagnosis was: Ca mammae l. dex. Mastectomy and ordinary hormonal therapy came after. CONCLUSION: We reported on the case of a female patient with an atypical form of Herpes in whom endogenuos virus reactivation was caused by the immunocompromisied state due to malignant breast disease.
