Summary: Daily PEF amplitude reflects bronchial hyperactivity and its measurement and expression via indexes of variability is indirect estimation of this phenomenon characterising asthma. The aim of this study is to estimate the validity of some indexes of daily PEF variability (DV) in making asthma diagnosis. PEF values for DV calculations are obtained from a group of 68 children (35 in the period before asthma was diagnosed and 33 healthy children) during a three-week follow-up period. Ninety five percent of the total of 693 DV calculated values was below 17% in healthy children group. Even 99% was below either 22.23% or 24% depending on the mode of calculation. Only 5%, approximately 1 (1.11) result per participant, exceeded the borderline of 17%. Daily variability over 19.9%, presented twice during the three-week follow-up period with the probability of 83.87%, indicates asthma with relatively high sensitivity (74.29%) and accuracy (79.41%). Taking into account all the limitations of PEF measurement in asthma diagnosing, it is possible to conclude that DV PEF presence exceeding certain borderline which is specific for 95th percentile of healthy population and its recurrence in a short period of time indicates asthma with high probability in a group of children with asthma like symptoms.
