Unrecognized perforated appendicitis in the third trimester of pregnancy - Case report

      Summary:
Appendicitis is one of the most common acute surgical diseases in pregnancy. The incidence of appendicitis in pregnancy is about 1:1000-1:5000 and it is not different in general population, but perforated appendicitis occurs 2 to 3 times more frequently in pregnancy than in general population. This is a case report of a 29-year-old patient in the third trimester of pregnancy in whom acute appendicitis was unrecognized and complicated with perforation, but it was misunderstood as urinary infection. She was given antibiotic, spasmolytic and tocolytic therapy which contributed to cover the clinical picture of the disease. In our case cesarean section was done because of obstetric indications in the 37th week of gestation and perforated appendicitis with peritonitis was recognized and operated at the some time. A male child was born alive, with birth weight of 2650g, birth length of 48 cm and Apgar scor of 5/8. Differential diagnosis of acute appendicitis in pregnancy includes a great number of diseases with abdominal pain. Symptoms and signs of acute appendicitis in pregnancy and out of it are very different and it is caused by anatomical and physiological changes during pregnancy. A well-timed, right diagnosis of acute appendicitis in pregnancy is established in 50-64 % cases. In every case of abdominal pain in pregnancy, it has to be thought about the possibility of occurring of acute appendicitis. In cases of suspected appendicitis during pregnancy, laparoscopy is indicated before 20 weeks of pregnancy and laparotomy after that period.
