      Summary:
INTRODUCTION: Peripartal cardiomyopathy is a rare, life-threatening complication of previously healthy mothers-to-be, whose incidence is approximately 1 per 3000 to 4000 births [1.2]. Criteria for diagnosis include: heart failure resulting from the last month of pregnancy up to 5 months after delivery, the absence of well-known cause for the occurrence of heart failure, the absence of heart disease before the last month of pregnancy and a echocardiographically registered absence of left ventricular systolic dysfunction before pregnancy [3].
CASE REPORT: A 26-year old primiparous, delivered in 38 gestation week, on the second day after cesarean section there was a sudden came dispnoa, ortopnoa, fatigue and chest pain. After a few hours she noticed blood in sputum. After clinical and laboratory findings, x-ray and echocardiography, the diagnosis of dilated cardio-myopathy and mitral insufficiency of the third degree was set. After transferring to the intensive care unit and application of the appropriate therapy the symptoms withdrew.
CONCLUSION: Peripartal cardiomyopathy is a rare but life-threatening complication of pregnancy. Our goal is to emphasize the importance of early diagnosis and timely adequate treatment of patients during the peripartal period with suddenly occurred dispnoe, ortopnoe and blood in sputum.
