Summary: Various types of serum antibodies can be found as a consequence of alloimmunization in pregnan¬cy and a cause of hemolytic disease of newborn. The aim of this study was to determine what types of antibodies were the cause of positive Coombs test results and which of them had caused severe alloimmunization which led to the need for exsanguinotransfusion administration. Material and methods: occurrence of alloimmunization in pregnancy was analysed in the retrospective study performed in an 8 year period, from January 2003 to January 2011 at Blood Transfusion Institute in Vojvodina. The study included 742 pregnant women with positive Coombs test results. The presence of antigens was determined by use of ID cards on DiaMed's Swing TwinSampler Instrument. Afterwards the type and titer of antibody in serum of pregnant women were deter-mined. Results: positive indirect antiglobuline test due to presence of passive antibodies after prophylaxis with Rhogam was found in 76.01% (564) pregnant women. 22.24% of all pregnant women with positive Coombs test results were sensibilised with the following antibody distribution: 44.85% anti-D (74), 1.12% anti-D+C (20), 9.69% anti-M (16), 2.42% anti-E (4), 1.82% anti- C (3), 0.61% anti-D+E (1), 1.82% anti- K (3), 0.61% anti-S+K (1), 1.1% anti-S (2), 3.03% anti Lea(5) and 1.1% irregular antibodies (35). Conclusion: in most cases of alloimmu¬nization in pregnancy the most important role, in spite of prophylaxsis with Rhogam, still plays RhD blood group system; nevertheless other less frequent antibodies have huge individual significance as well.
