The examination of platelet function and efficiency of antithrombotic therapy in patients with acute stroke
(Ispitivanje funkcije trombocita i efikasnosti antitrombocitne terapije kod akutnog ishemijskog moždanog udara (AIMU))

Introduction: acute ishemic stroke (AIS) is defined as a focal or global disturbance of brain function that occurs suddenly, lasts longer than 60 minutes, and results in a cerebrovascular disorder or a condition in which blood flow is not sufficient to meet the metabolic needs of neurons for oxygen and glucose. The incidence of acute ishemic stroke (acute ischemic stroke) varies in different countries and amounts to about 100 to 300 new cases per 100,000 people per year. Acute ischemic stroke tends to recur, the recurrence rate at the end of the first year being 10-18%, at the end of the second year 16-26%, and after the third year 20-34%. Thus, secondary prevention of acute ishemiskog stroke (acute ischemic stroke) is of paramount importance. In secondary prevention, an important role is played by antiplatelet drugs. Aim: testing the platelet function and efficiency of the treatment in patients with acute ischemic stroke, being on single or double antiagreggation therapy. Material and methods: a retrospective study was conducted on 45 patients who were on monotherapy or combined antiplatelet drug therapy. The data from the database of the Neurological Department of the Health Care Centre Zaječar was used, regarding the patients hospitalized in the period April-end October 2012. Results: of the total number of respondents 22 (48.9%) were men and 23 (51.1%) women. The average age was 69.02 (SD 11.297). The platelets function was studied in 45 patients. The physiological function of platelets had 30 (66.7%), while the non-physiological function of platelets had 15 (33.3%) patients. The effect of acetylsalicylic acid (ASA) was examined in 42 patients. ASK satisfactory effect was achieved in 23 (54.8%) patients, of which 13 (31.0%) were men and 10 (23.8%) women, and the effect was unsatisfactory in 19 (45.2%) patients, of which 8 (19.0%) were men and 11 (26.2%) women. The effect of clopidogrel was studied in 23 patients. A profound effect of clopidogrel was achieved in 13 (56.5%) patients, of which 8 (34.8%) male and 5 (21.7%) female, and the unsatisfactory effect was noted in 10 (43.5%) patients - 5 (21.7%) male and 5 (21.7%) female. Conclusion: the possibility of monitoring the platelet response to ASA and clopidogrel therapies can have a major impact on the treatment and a significant reduction in morbidity and mortality rates. It should be noted that the small sample of this study is a lack, and in order to define the impact of resistance to the antiplatelet therapy it is necessary to implement a more extensive research.
