| Summary: Patients with end-stage renal disease treated with hemodialysis have a high mortality rate. Hyperkalemia, volume overload, metabolic acidosis and secondary hyperparathyroidism are likely to play a cru¬cial role in triggering the ectopic impulses of the heart and may lead to sudden cardiac death in these patients. The potential risk factors for the occurrence of sudden cardiac death include angiotensin II-induced electrical remodeling of the myocardium, increased sympathetic activity and prolonged QT interval. Strategies to decrease sudden cardiac death in patients treated with dialysis include primary and secondary prevention. The primary prevention of sudden cardiac death includes a medicament therapy and changes in the dialysis prescription, and the secondary prevention includes coronary revascularization and the use of implantable cardioverter-defibrillators.
