| Summary: Acute pancreatitis (AP) is a disease with a large spectrum of morphological and clinical findings, unpredictable course and uncertain outcome. Regardless of modern diagnostic and therapeutic procedures, the mortality rate in severe disease still reaches 50%. Pathophysiology of the disease is not yet fully clarified and the appropriate and successful treatment option is still not known. The treatment of AP must be multidisciplinary, which mainly comes down to a rapid and accurate assessment of the disease severity, as well as continuous monitoring of patients. For the evaluation of local and systemic inflammation in the disease, for more than a century there have been different scoring systems, clinical and laboratory parameters. None of these markers of the disease severity is entirely accurate and sensitive when used alone. Knowledge of all parameters that may indicate severe disease and poor outcome and their continual usage are key factors for the adequate and timely treatment of AP.
