Summary: A twenty-one-year old patient, who had been suffering from "hot water epilepsy" (HWE) since he was seven, comes to see his epileptologist for a regular check. The attacks occur as complex partial seizures with occasional secondary generalization, and generally are precipitated by bathing in warm water. They started with the carbamazepine therapy, which achieved good seizure control, but because of the adverse effects, they had to change to valproates, and then on lamotrigine. Because of occasional deterioration, the drug combination was changed, topiramate was introduced, and levetiracetam later on. The clinical condition varied with such a therapy and eventually led to pharmacoresistance. Interictal EEG showed occasional epileptogenic activity. Given that this was a young man with a complex disease, further diagnostic and therapeutic aspect must be multidisciplinary.
